Documents - Please read carefully

  1. The below Permission to Attend form does not need to be filled out in advance if the attending student is 18 on or before June 14, 2023. But they will be required to sign the agreement at check-in on the night of the event.
  2. If the attending student will not be 18 before June 14, 2023, the below form must be filled out in advance by a parent or guardian.  Print the Permission to Attend form, fill it out, and scan back to bellevuehigh2023@outlook.com. Please put the student's name in the subject line.
  3. If the student has a life-threatening condition and will need medication available during the event, please fill out the Emergency Care Plan document below. (Only needed if applicable.) The attending student will need to bring the completed form along with their medication to the Grad Night Party.
  4. Once the documents have been reviewed, signed, and submitted (if needed) please go to the BUY TICKETS tab to purchase your ticket.

The Life-Threatening Emergency Care Plan is only required if the student has a life-threatening medical condition. Please download and bring this completed form with you to the Grad Night Party, along with any medication in a zip lock bag that is labeled with the student's name and any instructions for dispensing. This medication will be given to the Grad Night Chaperones at check-in and will be returned to you at the end of the night.
